Director Saran's movies have always been the relatable films and family appropriate. Having directed Aasal in 2010, he returned to Tamil cinema with Aayirathil Iruvar  in 2017. Post that, it was announced that the Vasool Raja director is collaborating with the Bigg Boss fame Aarav, titled as "Market Raja MBBS" and the first look astonished everyone. Having featured the stellar actor Radhikaa Sarathkumar, the versatile actress Rohini has been roped into the movie to play the role of Lathamma, a mother with speech and hearing impairment. It's been said that the role is an exceptional one with a wider scope of acting.

In a recent interview director Saran had said that whilst he began narrating the script and her role, he had observed Rohini inheriting the qualities of Latha. He added that after quite some time, she'd asked him to narrate the script and converse with her in sign language.Saran also stated that the actress had breathed life into the character and that even after the wrap, she'd take longer to step out of her character.

Apart from the aforementioned actors, Market Raja MBBS  features Kavya Thapar as the leading lady; Nassar, Hareesh Peradi, Yogi Babu, Pradeep Rawat, Sayaji Shinde, Nikesha Patel, Devadarshini, Chams, Adhithya Menon and Munishkanth are appearing in pivotal roles.
